fa عفونت‌های قارچی دستگاه عصبی مرکزی - بخش اول Fungal infections of the CNS - Section 1 قارچ شناسی قارچ شناسی مروری Review <div style="text-align: justify;">گونه‌های کاندیدا اگر چه از پاتوژن های نادر سیستم اعصاب مرکزی هستند، می‌توانند در مننژها و بافت پارانشیمال مغز ایجاد عفونت کنند. اکثر موارد کاندیدیازیس سیستم اعصاب مرکزی با کاندیدیازیس تهاجمی یا منتشره در ارتباط است. فرم دیگر عفونت کاندیدایی سیستم اعصاب مرکزی به صورت عوارض بعد از عمل روش‌های نوروسرجیکال به ویژه قرار دادن شانت ونتریکولوپریتونئال است. حدود نیمی از بیمارانی که در اثر کاندیدیازیس تهاجمی می‌میرند مدارکی دال بر گرفتاری CNS دارند. اغلب اوقات عفونت‌های کاندیدا مننژها را مبتلا می‌سازد، هر چند که آبسه‌های داخل جمجمه‌ای می‌تواند به صورت ایزوله و یا مرتبط با مننژیت نیز رخ بدهد. آبسه‌ها معمولاً به صورت میکرو آبسه‌های کوچک متعدد هستند و در افراد با سیستم ایمنی مختل که به عفونت منتشره مبتلا شده‌اند دیده می‌شوند. مننژیت کاندیدایی به صورت تظاهری از کاندیدیازیس منتشره اغلب در نوزادان نارس در حضور دستگاه‌های درناژ ونتریکولار و نیز به صورت یک مننژیت مزمن ایزوله دیده می‌شود. علاوه بر انتشار از طریق جریان خون کاندیدا می‌تواند در زمان انجام کرانیوتومی و یا از طریق یک شانت ونتریکولار وارد CNS شود. مننژیت کاندیدایی اغلب اوقات در نوزادان نارس به دنبال روش‌های جراحی اعصاب پدید می‌آیند، علاوه بر این بیمارانی که به عفونت‌های سیستم اعصاب مرکزی دچار می‌شوند اغلب اختلال سیستم ایمنی دارند. بیمارانی که در مولکول آداپتور گیرنده لکتین CARD9 نقص یا کمبود دارند به ویژه نوزادان و اطفال ریسک ابتلا به کاندیدیازیس سیستم اعصاب مرکزی در آن‌ها بیشتر است. به جز کاندیدا آلبیکنس گونه‌های دیگر نظیر پاراپسیلوزیس، تروپیکالیس و گلابراتا به عنوان عوامل نادر عفونت‌های CNS شناخته شده‌اند. کریپتوکوکوس نئوفرمنس فراوان‌ترین عامل مننژیت قارچی است. بیماری هودجکین، لنفوم، سارکوئیدوز و لنفوپنی CD4 ایدیوپاتیک از جمله شرایطی هستند که استعداد به کریپتوکوکوزیس را ایجاد می‌نمایند. کورتیکواستروئیدها و درمان برای پیوند بافت‌های جامد نیز فرد را مستعد به این عفونت می‌کند.</div> <div style="text-align: justify;">While fungal infections of the CNS are relatively rare, they have become more common with the increasing number of individuals who are immunocompromised due to HIV/AIDS, immunosuppressive therapies, invasive diagnosis and treatment methods, and organ transplants. CNS fungal infections present many diagnostic and therapeutic challenges and are associated with a high mortality rate. <a name="_Hlk122688079">Cryptococcal meningoencephalitis </a>is the most common of these infections and usually affects patients with uncontrolled <a href="https://www.neurologyadvisor.com/home/topics/general-neurology/aging-related-brain-changes-no-different-in-hiv-positive-negative/" style="color:blue; text-decoration:underline" target="_self">HIV infection</a>. While immunocompromised patients are the most susceptible to CNS fungal infections, they can also occur in immunocompetent patients undergoing invasive procedures such as neurosurgery and in patients exposed to contaminated devices or drugs.  In addition, heavy exposure to fungi in endemic regions can lead to infection in immunocompetent individuals. CNS infection due to Cryptococcus species is most often caused by Cryptococcus neoformans, with C gattii as a less common cause, and often arises from a primary lung infection. Meningoencephalitis is a typical feature of <a name="_Hlk122688208">Cryptococcus infection </a>of the <a href="https://www.neurologyadvisor.com/home/topics/general-neurology/cns-abnormalities-common-in-erdheim-chester-disease/" style="color:blue; text-decoration:underline" target="_self">CNS</a>. Common signs and symptoms include headache, nausea, vomiting, altered mental status, impaired vision, and 6th nerve palsy. In patients with uncontrolled HIV infection, symptoms might be absent or subtle before initiation of antiretroviral therapy. Candida-related CNS infections are usually caused by Candida albicans, arise from hematogenous spread, and present with overt meningitis. These cases may involve chronic meningitis, brain <a name="_Hlk122688388">abscesses</a>, vasculitis with cerebral infarctions, spinal infections, ventriculitis, and mycotic aneurysms. Candida meningitis and meningoencephalitis are uncommon.
