fa مروری بر آخرین معیارهای کلینیکی و آزمایشگاهی در تشخیص سندرم شوگرن A review on the latest clinical and laboratory criteria for diagnosis of the Sjogren’s syndrome ایمنی شناسی (ایمونولوژی) ایمنی شناسی (ایمونولوژی) مروری Review <div style="text-align: justify;">سندرم شوگرن ((SS)Sjogren’s syndrome) یک بیماری اتوایمیون مزمن پیشرونده است که معمولاً توسط روماتولوژیست ها پیگیری می‌شود. تشخیص تأخیری بیماری شایع است به این دلیل که علائم بیماری غیر اختصاصی و پیشرفت بیماری کند است. مشخصه ویژه بیماری SS خشکی چشم و دهان است هر چند که طیف وسیعی از علائم مانند درد مفاصل، راش‌های پوستی، سرفه‌های خشک مزمن، خشکی واژینال، بی حسی در اندام‌های انتهایی و خستگی طاقت فرسا را هم شامل می‌شود. معیارهای تشخیصی رایج که توسط انجمن روماتولوژی آمریکایی/ اروپایی در سال 2016 (2016 ACR/EULAR) ارائه شده می‌تواند در تشخیص SS کمک کننده باشد. این معیارها شامل علائم بالینی، مثبت شدن تست‌های سرولوژی و همچنین یافته‌های بیوپسی از غدد بزاقی می‌باشد. بیومارکرهای رایج که برای بیماری SS استفاده می‌شود شامل anti-SS-A /Ro،ANA, anti SS-B/La و RF می‌باشد. در حالی که این بیومارکرها جهت تشخیص بیماری مفید است همیشه به خصوص در اوایل بیماری مثبت نیستند و علاوه بر این اختصاصی بیماری SS نیز نمی‌باشند. چندین بیومارکر که اختصاصی غدد بزاقی - اشکی هستند جدیداً معرفی شده‌اند که شامل: SP-1 (salivary gland prolein-1) (parotid secretory protein) PSP و (Carbonic anhydrase) CA-6 می‌باشند. مطالعات نشان می‌دهد که این بیومارکرهای جدید در تشخیص زود به هنگام بیماری در مراحل اولیه بسیار حائز اهمیت هستند. به همین دلیل محققان تستی را طراحی کردند به اسم test SjO diagnostic که به طور تجاری جهت تشخیص بیماری SS در دسترس می‌باشد و در این تست از بیومارکرهای رایج (anti-SS- A/RO, anti-SS-B/La, ANA and RF) و همچنین بیومارکرهای جدید (antibodies to SP-1, PSP and CA-6) استفاده شده است. این تست حساسیت و اختصاصیت تشخیص SS را افزایش می‌دهد. تشخیص به موقع بیماری SS نیازمند همکاری روماتولوژیست ها، چشم پزشکان و دندان پزشکان و انتخاب درست differential work-up که شامل ارزیابی‌های فیزیکی و بالینی می‌باشد.  </div> <div style="text-align: justify;">Sjogren's syndrome (SS) is a chronic progressive autoimmune disease that is usually followed by rheumatologists. Delayed diagnosis of the disease is common because the symptoms of the disease are non-specific and the disease progresses slowly. SS is characterized by dry eyes and mouth, although a wide range of symptoms such as joint pain, skin rashes, chronic dry cough, vaginal dryness, numbness in the extremities, and excruciating fatigue are also included. The common diagnostic criteria proposed by the American/European Society of Rheumatology in 2016 (2016 ACR/EULAR) can help in the diagnosis of SS. These criteria include clinical symptoms, positive serology tests, and biopsy findings of salivary glands. Common biomarkers used for SS disease include anti-SS-A/Ro, ANA, anti SS-B/La and RF. While these biomarkers are useful for diagnosing the disease, they are not always positive, especially in the early stage of the disease, and importantly, they are not specific for SS disease. Several biomarkers specific to salivary-lacrimal glands have been recently introduced, including SP-1 (salivary gland prolein-1) (parotid secretory protein) PSP and (carbonic anhydrase) CA-6. Studies show that these new biomarkers are very important in the early diagnosis of the disease. For this reason, the researchers designed a test called SjO diagnostic test, which is commercially available to diagnose SS disease, and in this test, common biomarkers (anti-SS-A/RO, anti-SS-B/LO, ANA and RF) and new biomarkers (antibodies to SP -1, PSP and CA-6) are evaluated. This test increases the sensitivity and specificity of SS diagnosis.
