fa بررسی ارتباط بین فارماکوژنومیکس و داروهای قلبی عروقی Exploring the Relationship Between Pharmacogenomics and Cardiovascular Drugs ژنتیک ژنتیک مروری Review <div style="text-align: justify;">بیماری‌های قلبی عروقی<a href="#_ftn1" name="_ftnref1" title="">[1]</a>  (CVD) یکی از نگرانی‌های عمده سلامت جهانی است که منجر به نرخ بالای مرگ و میر می‌شود. اثربخشی و ایمنی درمان‌های CVD می‌تواند به دلیل عوامل ژنتیکی فردی متفاوت باشد. این مقاله به بررسی این موضوع می‌پردازد که چگونه فارماکوژنومیک می‌تواند بر ایمنی و اثر بخشی درمان‌های قلبی عروقی از طریق روش‌های آزمایش ژنتیک پیشرفته تأثیر بگذارد. CVD شامل شرایطی است که بر قلب و گردش خون تأثیر می‌گذارد، مانند بیماری عروق کرونر قلب، بیماری عروق محیطی و بیماری عروق مغزی. عوامل ژنتیکی نقش مهمی در تغییر پذیری پاسخ‌های دارویی در درمان‌های CVD دارند. رویکردهای درمانی شخصی برای مدیریت مؤثر با در نظر گرفتن عوامل ژنتیکی، محیطی و شیوه زندگی بسیار مهم هستند. فارماکوژنتیک برای پزشکی دقیق در مراقبت از قلب و عروق ضروری است. عوامل ژنتیکی به طور قابل توجهی بر اثر بخشی و ایمنی داروهای قلبی عروقی تأثیر می‌گذارد و هزینه‌های درمان را کاهش می‌دهد. تغییر پذیری در پاسخ به دارو تحت تأثیر عواملی مانند جذب، متابولیسم و تعامل با گیرنده‌ها است. این بررسی بر نقش آزمایش ژنتیکی در بهینه سازی اثر بخشی و ایمنی دارو در پزشکی قلبی عروقی دقیق تمرکز دارد. جفت‌های کلیدی ژن-دارو با شواهد قوی برای اجرای بالینی شامل کلوپیدوگرل و ژنوتیپ CYP2C19، وارفارین و ژنوتیپ های CYP2C9 و VKORC1 و ژنوتیپ سیمواستاتین و SLCO1B1 است. تغییرات ژنتیکی نیز بر پاسخ به مسدود کننده‌های β تأثیر می‌گذارد. مؤسسات در حال ادغام آزمایش فارماکوژنومیک این جفت‌های دارو-ژن در عمل بالینی برای بهبود نتایج بیمار هستند. یک رویکرد پیشگیرانه برای آزمایش فارماکوژنومیک، به جای شروع پس از درمان، می‌تواند برای پزشکان و بیماران مفید باشد. این بررسی بر فارماکوژنتیک داروهای قلبی عروقی مانند کلوپیدوگرل، وارفارین و سیمواستاتین تاکید می‌کند که توسط شواهد بالینی قوی پشتیبانی می‌شود. چالش‌های تبدیل فارماکوژنتیک به عمل بالینی شامل نیاز به داده‌های نتیجه کارآزمایی های مبتنی بر ژنوتیپ است. کارآزمایی های تصادفی و کنترل شده برای ارزیابی سودمندی بالینی استراتژی‌های مبتنی بر ژنوتیپ در حال انجام است. مراکز پزشکی دانشگاهی در خط مقدم اجرای فارماکوژنتیک قرار دارند و به الزامات عملیاتی برای ادغام موفقیت آمیز سرپرستی می‌کنند.</div> <div style="text-align: justify;"></div> <div style="text-align: justify;"></div> <div style="text-align: justify;">Cardiovascular diseases (CVD) are major global health concerns that contribute to high mortality rates. The efficacy and safety of CVD treatments can vary significantly due to individual genetic factors. This article explores how pharmacogenomics can impact the safety and effectiveness of cardiovascular treatments through advanced genetic testing methods. CVD includes conditions affecting the heart and blood vessels, such as coronary artery disease, peripheral artery disease, and cerebrovascular disease. Genetic factors play a key role in the variability of drug responses in CVD treatments. Personalized therapeutic approaches, considering genetic, environmental, and lifestyle factors, are crucial for effective management. Pharmacogenomics is essential for precision medicine in cardiovascular care. Genetic factors significantly influence the efficacy and safety of cardiovascular drugs and help reduce treatment costs. Variability in drug response is influenced by factors such as absorption, metabolism, and receptor interactions. This review focuses on the role of genetic testing in optimizing the efficacy and safety of cardiovascular drugs in precision medicine. Key drug-gene pairs with strong clinical evidence for implementation in practice include clopidogrel and the CYP2C19 genotype, warfarin and the CYP2C9 and VKORC1 genotypes, and simvastatin and the SLCO1B1 genotype. Genetic variations also affect response to β-blockers. Institutions are integrating pharmacogenomic testing of these drug-gene pairs into clinical practice to improve patient outcomes. A proactive approach to pharmacogenomic testing, rather than starting after treatment, could be beneficial for both physicians and patients. This review emphasizes the pharmacogenetics of cardiovascular drugs such as clopidogrel, warfarin, and simvastatin, which are supported by strong clinical evidence. Challenges in translating pharmacogenomics into clinical practice include the need for genotype-based trial outcome data. Ongoing randomized controlled trials aim to assess the clinical benefits of genotype-based strategies.
