fa چه موقع وقت آن است که خود را از دست قوانین هشدار دهنده خلاص کنید؟ علوم آزمایشگاهی علوم آزمایشگاهی مروری Review <div style="text-align: justify;"><span style="font-size:14px;"><span style="font-family:Tahoma;"><span style="line-height:normal"><span style="direction:rtl"><span style="unicode-bidi:embed"><span lang="FA">طی چندین دهه اخیر، روش ‌های آماری برای پایش کیفیت (</span><span dir="LTR">QC</span><span lang="FA">) عملکرد سنجشی در آزمایشگاه پزشکی ظهور و توسعه یافته‌ اند. یکی از نخستین گام‌ ها در بهبود طراحی </span><span dir="LTR">QC</span><span lang="FA"> این بود که با جایگزین شدن نرم ‌افزارها برای تفسیر قوانین </span><span dir="LTR">QC</span><span lang="FA"> و برداشته شدن این وظیفه از دوش کاربران، دیگر لازم نبود قانون </span><span dir="LTR">s1:2</span><span lang="FA"> به عنوان قانون هشدار استفاده شود و این قانون از دستور پایش &laquo;چند قانونی وستگارد&raquo; حذف شد. یک گام دیگر شخصی ‌سازی برنامه </span><span dir="LTR">QC</span><span lang="FA"> بر اساس کیفیت عملکرد (که با عیار سیگما بیان می ‌شود) بود. حاصل این رویکرد این بود که دیگر استفاده از همه قوانین وستگارد برای همه عملکردها توصیه نمی‌ شد بلکه با افزایش کیفیت عملکرد (عیار سیگما)، قوانین کمتر و نتایج کنترل کمتری لازم است. به رغم این تحولات، برخی آزمایشگاه‌ ها همچنان به اشتباه به استفاده از قوانینی اضافی و سنجش تعداد اضافی کنترل ادامه می ‌دهند که تنها حاصل آن اضافه کردن احتمال رد کاذب است. برای رهایی از این رویکرد اشتباه، لازم است آزمایشگاهیان با مبانی طراحی </span><span dir="LTR">QC</span><span lang="FA"> آماری آشنا شوند و نیز بر هراس نابجای کاستن از قوانین اضافه غلبه بکنند.</span></span></span></span></span></span></div> <div style="text-align: justify;"><span style="font-size:11pt"><span style="line-height:normal"><span style="font-family:Calibri,sans-serif"><span style="font-size:12.0pt"><span style="font-family:&quot;Times New Roman&quot;,serif">Over several recent decades, statistical quality control (QC) procedures to control the quality of analytical performance in clinical laboratory have emerged and improved. Of the first steps in improving QC planning happened with the replacement of software in the QC interpretation and thereby getting the operators off this duty so that no longer it was necessary to use 1:2s rule as a warning rule and this rule was removed from Westgard Multirule procedure. Another step was individualizing QC based on the performance&#39;s quality (expressed as Sigma metric). This approach resulted in that no longer it was recommended to use all the Westgard rules for all the performances but aligned with the increase of the performance&#39;s quality (Sigma metric), less rules and less control results are needed. Despite these improvements, some laboratories are still using excessive rules and excessive number of control assays that leads only to the increase of false rejection.
