fa عفونت‌های قارچی در بیمار مبتلا به نقص ایمنی-بخش پنجم Fungal Infections in the Immunocompromised Patient-Section 5 قارچ شناسی قارچ شناسی مروری Review <div style="text-align: justify;">گونه‌های کاندیدا شایع‌ترین علل عفونت قارچی در گیرنده پیوند عضو هستند و طیف وسیعی از سندرم‌ها را از موارد جزیی و سطحی تا موارد مهلک ایجاد می‌کنند. شیوع بیمارستانی گونه‌های کاندیدا به دست‌های آلوده پرسنل بیمارستان نسبت داده شده است. عفونت کاندیدایی مری در افراد دارای نقص ایمنی، یافته شایعی است و اغلب با عفونت دهانی- حلقی مرتبط می‌باشد. تشخیص با ارزیابی آندوسکوپی از جمله بیوپسی انجام می‌شود. از آنجا که آسیب‌های مربوط به ازوفاژیت هرپسی و سایتومگالوویروس و نیز آثار پرتودرمانی و ریفلاکس در رادیوگرافی و حتی آندوسکوپی علایم و نشانه‌های مشابهی ممکن است داشته باشند رویکرد تشخیصی دقیقی لازم است انجام شود. باید توجه داشت که ازوفاژیت کاندیدایی نیاز به درمان ضد قارچی سیستمیک دارد و درمان موضعی بی اثر است. کاندیدیازیس دستگاه ادراری نیز در بیماران پیوند عضو به دلیل ابزار گذاری در دستگاه ادراری، دیابت، عفونت ولوواژینال و وجود مثانه نوروژنیک امکان پذیر است. اکثریت قریب به اتفاق موارد عفونت دستگاه ادراری از طریق مسیر صعودی رخ می‌دهد. کاندیدمی و عفونت منتشر کاندیدایی بیشتر به دلیل عفونت کاتترهای دسترسی عروقی رخ می‌دهد، اگر چه انتقال از طریق  روده‌ای در بیماران بد حال نیز می‌تواند رخ دهد. پنومونی کاندیدایی از طریق مسیر تراکئوبرونشیال به ندرت اتفاق می افتد، در حالی که تهاجم ریوی ناشی از عفونت خونی به خوبی تشخیص داده می‌شود. این گفته حتی زمانی که خلط در آزمایش میکروسکوپی و کشت، اشکال کاندیدایی قابل مشاهده‌ای داشته باشد و حتی زمانی که کشت خون مثبت باشد صادق است. گونه‌های آسپرژیلوس ساپروفیت های فراگیر در محیط  هستند و فرم عفونی یعنی کونیدیا از راه استنشاق وارد دستگاه تنفسی شده و بیماری را آغاز می‌کند. پس از استنشاق، این کنیدیاها می‌توانند سندرم‌های بالینی متنوعی ایجاد کنند که در چهار دسته تحت عناوین سندرم‌های حساسیت بیش از حد، سندرم‌های کلونیزاسیون، آسپرژیلوزیس نیمه تهاجمی و آسپرژیلوزیس مهاجم دسته بندی می‌شوند. مواجهه‌های بیمارستانی هنگام تردد بیماران در بیمارستان برای انجام اقدامات ضروری رخ می‌دهد. بنابراین، شیوع بیماری یا موارد منفرد ناشی از مواجهه در اتاق عمل آلوده، اتاق رادیولوژی یا در حین انتظار برای برونکوسکوپی یا بیوپسی اندومیوکاردیال در اتاق انتظار ثبت شده است. آسپرژیلوس فومیگاتوس مسئول حدود 90٪ موارد بیماری‌های تهاجمی است و آسپرژیلوس فلاووس اکثر موارد باقی مانده، به ویژه مواردی که سینوس‌های پارانازال را درگیر می‌کنند تشکیل می‌دهد. آسپرژیلوس ترئوس، آسپرژیلوس نیدولانس و آسپرژیلوس نیجر، گاهی اوقات بیماری ایجاد می‌کنند و شواهدی وجود دارد که این عفونت‌های غیر معمول، به ویژه در بین گیرندگان پیوند ریه در حال افزایش است. این گونه‌های غیر معمول تمایل بیشتری به مقاومت دارویی دارند. برای مثال آسپرژیلوس ترئوس نسبت به آمفوتریسین مقاوم است. ویژگی کلیدی آسپرژیلوزیس مهاجم، صرف نظر از محل ورود، رفتار تهاجمی آن به رگ‌ها است. سه ویژگی اصلی آسپرژیلوزیس مهاجم عبارت از خونریزی، انفارکتوس و متاستاز می‌باشد.  </div> <div style="text-align: justify;">Candida species are the most common causes of fungal infection in organ transplant recipients and cause a wide range of syndromes from minor and superficial cases to fatal ones. Nosocomial outbreaks of Candida species have been attributed to contaminated hands of hospital personnel. Esophageal candidiasis is a common finding in immunocompromised individuals and is often associated with oropharyngeal infection. Diagnosis is made by endoscopic evaluation, including biopsy. Since lesions related to herpetic and cytomegalovirus esophagitis, as well as the effects of radiation therapy and reflux, may have similar signs and symptoms on radiographs and even endoscopy, a careful diagnostic approach is necessary. It should be noted that candidal esophagitis requires systemic antifungal therapy and topical therapy is ineffective. Urinary tract candidiasis is also possible in organ transplant patients due to urinary tract instrumentation, diabetes, vulvovaginal infection, and the presence of a neurogenic bladder. The vast majority of urinary tract infections occur via the ascending route. Candidemia and disseminated candidal infection are most commonly due to infection of vascular access catheters, although enteric transmission can also occur in critically ill patients. Candida pneumonia via the tracheobronchial route is rare, whereas pulmonary invasion from bloodstream infection is well recognized. This is true even when sputum contains visible Candida species on microscopy and culture and even when blood cultures are positive.  Aspergillus species are widespread saprophytes in the environment, and the infectious form, conidia, enters the respiratory tract through inhalation and initiates the disease. After inhalation, these conidia can cause a variety of clinical syndromes that are grouped into four categories: hypersensitivity syndromes, colonization syndromes, semi-invasive aspergillosis, and invasive aspergillosis. Hospital-acquired exposures occur as patients move around the hospital for essential procedures. Therefore, outbreaks or isolated cases resulting from exposure in a contaminated operating room, radiology room, or while waiting in a waiting room for bronchoscopy or endomyocardial biopsy have been documented. Aspergillus fumigatus is responsible for about 90% of invasive disease cases, and Aspergillus flavus accounts for most of the remaining cases, especially those involving the paranasal sinuses. Aspergillus tereus, Aspergillus nidulans, and Aspergillus niger sometimes cause disease, and there is evidence that these unusual infections are increasing, particularly among lung transplant recipients. These unusual species tend to be more resistant to drugs. For example, Aspergillus tereus is resistant to amphotericin. The key feature of invasive aspergillosis, regardless of the site of entry, is its invasive behavior in the blood vessels.
